Initiative and Candidate Petition Affidavits
In Colorado, people who circulate petitions to put initiatives or candidates on the election ballot must sign an affidavit on each section of the petition. The circulator must sign the petition in the presence of a notary public.
The procedure is explained on the Secretary of State's website here.
